Does anyone know this how to download/configure this theme?

Like the FreeBSD icon on KDE Menu, the colors, and the others icons.

have you looked at kde-look.org?
maybe they have it there.

AS LatenightTv suggested, head to kde-look.org, and grab an icon theme you are interested in. There are a lot of fantastic icon themes available for 3.5.10, that haven't been ported yet for 4.2.

As far as themes go, again follow LatenightTV's advice and look up kde-look.org for 3.5.10 themes. In my opinion, the domino style for 3.5.10 is pretty slick, and there are a lot of domino styles available that are simply great.

The icon theme is FreeBSD mod of Crystal Diamond (http://kde-look.org/content/show.php/Crystal+Diamond+Icons?content=45576).
